2012–2013 Graduate Student Health Insurance FAQ

1. May I purchase health insurance while attending graduate school at Cedarville University?

Graduate students may purchase health insurance. This is a limited health insurance plan. It is not designed to be the student’s only health insurance.

2. How much does the insurance cost and where do I purchase it?

The graduate student health insurance plan is $251 per semester. Enrollment forms are available at the student insurance office in UMS.

3. When am I covered under the graduate student health insurance plan?

Students are covered a semester at a time. Fall Semester starts August 15, 2012, and terminates midnight January 6, 2013. Insurance coverage for Spring semester starts January 7, 2013, and terminates midnight August 14, 2013.

4. Is the graduate student health insurance plan good only while I am on campus?

No. The student health insurance plan is good 24 hours a day, anywhere in the world.

5. What kind of health insurance is the graduate student health insurance plan?

It is a blanket policy, and it is a limited medical insurance plan. It is not designed to be the student’s only insurance.

6. How do I prove that I have the graduate student health insurance plan?

Insured students will receive an insurance card around the fourth week of the semester in their campus mailbox. If an insurance card is needed before that time, the student may pick one up at the insurance window in University Medical Services (UMS).

7. How does the graduate student health insurance plan work with my private insurance?

Your private insurance must make the decision as to which insurance is primary. Call your private insurance customer service department and give them the following information:

  1. I have student insurance with Cedarville University.
  2. The student insurance is NOT a major medical policy.
  3. The University is the policyholder.
  4. The student health insurance is a blanket policy.
  5. The student insurance does coordinate benefits.
  6. The student health insurance plan is a primary policy which may function as secondary when other insurance is available to the insured student.

8. What do I do with the information about which insurance is primary?

It is essential that you communicate to the student insurance coordinator which insurance is primary (pays first). The other insurance will be secondary (pays second).

9. Do I need a copy of my private medical insurance card(s)?

Yes. You need a card(s) of your own OR a copy of both sides of each private medical insurance card under which you are covered. Your insurance card(s) should be on your person at all times. UMS needs a copy of both sides of your private medical insurance card(s).

10. Does the graduate student health insurance plan cover motor vehicle accidents?

Yes, but only as a secondary policy after other insurance has paid first.

11. How do I use graduate student health insurance?

If you use your student health insurance, you must fill out a claim form. Medical bills covered by the graduate student insurance plan cannot be processed without an insurance claim form signed by you. Claims for medical bills are filled out in the student health insurance office at UMS. When your medical bills arrive at UMS, you will be notified by a note in campus mail, phone, or e-mail. If you receive a medical bill or a statement from a medical provider, just bring it to the student health insurance coordinator right away so the bill can be processed. Medical bills can only be paid after you have filled out a claim.

When you visit a medical provider off campus, show them your student health insurance card and your private insurance card. You will need to tell them which insurance is primary and which insurance is secondary. If you are not sure which insurance is primary, give the student insurance as primary. When the medical provider sends a bill to the student health insurance plan at UMS, as directed on the back of your insurance card, you will then be notified to come to UMS and fill out an insurance claim form. If you use your student health insurance during the summer, UMS staff will mail a claim form to your home upon receipt of the bill.

If you use your graduate student insurance for prescriptions, you will need to pay for them at the pharmacy. The pharmacy receipt, not the cash register receipt, is required by the insurance company for each prescription. You will be reimbursed within the scope of coverage.

Important note: The physician, hospital, urgent care facility, laboratory, or X-ray facility that provides you with medical care cannot be paid without your signature on the insurance claim form. Your prompt attention will help health care providers get paid in a timely fashion.

12. Can I insure my nonstudent spouse and/or my dependent children under the graduate student health insurance?

Yes. You must fill out a student health insurance plan enrollment form. Enrollment forms are available in the student health insurance office at UMS. The cost of the insurance will be added to your account and will vary from year to year.
