2009 FIT Challenge Evaluation Study -- Informed Consent

The purpose of this survey is to carefully evaluate the FIT Challenge in order to improve the program and best steward University resources used to support the challenge. Your participation in the study will involve completing on-line surveys at the beginning and end of the challenge.

Specifically, by gathering and analyzing this information, we hope:

  • To better describe the participants and non-participants in the FIT Challenge
  • To evaluate the benefits of participating in a campus fitness challenge
  • To determine if participation in a campus fitness challenge results in improved physical health and well-being

Your participation in this study is completely voluntary. You may discontinue participation at any time without penalty.

Your questionnaire responses will be kept confidential. Your pre and post-surveys will be linked using your Redwood ID number and not your name. Only the FIT Challenge coordinator will have access to your responses until it is grouped for analysis. It will not be individually reported to anyone else.

If reports about this study are published, your name will not be used and your information will not be individually identifiable.
