On behalf of the University Medical Services staff, welcome to Cedarville University!
University Medical Services (UMS) is the student health center at Cedarville University and exists to provide health care for you while you are away from home. University Medical Services is staffed with R.N.'s and part-time physicians who are available by appointment and on a walk-in basis. Most visits to UMS are free, except for labwork, prescriptions, and some medical equipment. You do not need insurance to use the services of UMS. In addition to the health care services available at UMS, all students taking six or more credit hours are enrolled in the Student Health Insurance Plan as a benefit of tuition. Information about the Student Health Insurance Plan can be found in the New Student Health Information booklet on pages 11 and 12 or at the Student Health Insurance Website.
What do you need to do?
- Complete the Cedarville University Student Health Form on pages 5 -10 of the New Student Health Information booklet and return it to:

Cedarville, OH 45314 - Please note the information regarding the measles (rubeola) immunity requirements in section 3 of the Student Health form and either return an official copy of measles immunity along with your Student Health form or have your physician complete section 3 of the form.
- Attach a front and back copy of your private insurance card to the Student Health form.
