Applying for Federal Loans

Please visit our Loans Overview page for applying for the Stafford, PLUS, Perkins, Nursing, or Alternative loan.

You must complete the Parent Request Form online if you desire to take out the PLUS loan. Our office can provide you with a paper copy upon request.

Institutional Loan Application

To be completed by students who want to apply for either Short-term or Long-term loans. Maximum of $2400.

To be used to determine some measure of ability to pay for international students and determine eligibility for financial assistance as applicable.

To be used by students to prove Ohio residency. This form only needs to be completed once.
