The information received through this application will assist the department of community ministries in evaluating your interest in a student leadership position and in determining your placement in the community ministries program. Please fill out this form completely!

Before you complete this form:

  1. Read the Community Ministries Team Leader Manual (includes a one-page summary of the Expectations and Responsibilities for Community Ministries student team leaders).

After you complete this form:

  1. Make sure that all boxes are completed. If a question is not applicable, please put N/A in the appropriate box.
  2. Submit completed form to community ministries by clicking on the button at the bottom of this page.
  3. Please submit this form within seven days as any delays may hinder your progress in the community ministries program.
