W-2

If you wish to receive an electronic copy of your W-2 instead of a paper copy, you will need to consent to receiving an electronic copy through the Self Service portal. View Instructions for performing this task.

What is a 1095-C?

The form 1095-C includes information about the health insurance coverage offered to you by Cedarville. Form 1095-C Part II includes information about the coverage, if any, the University offered to you, your spouse and dependent(s). Part III lists the covered individuals. This form is necessary to file your taxes.

Who receives this form?

All full-time employees of Cedarville for one or more months within a calendar year.

How will this form be sent?

  • Current employees can consent to electronic delivery through the Self Service portal. Once electronic delivery has been chosen, no paper forms will be produced and you will continue to have access to current and historic 1095-C's unless you withdraw your consent. 
  • For employees who have not consented to electronic delivery, a paper copy will be forwarded via campus mail by January 31. 
  • All former University employees will receive a paper form delivered via first class U.S. mail to the most current address on file.

What if my name or the name of my dependent(s) and Social Security Number is incorrect?

If you find your name and/or Social Security number is incorrect or your dependent(s), please contact Tax Forms as quickly as possible. Payroll will correct and reissue your 1095-C. You will be contacted when the correct 1095-C is available for you.

What if the wrong address is listed on my 1095-C?

If the wrong address is listed on your 1095-C, a corrected copy will not be created or distributed. Please update your address information in CedarInfo.

What if I need a replacement?

Employees can either consent to electronic viewing of the document via the Self Service portal or contact Tax Forms to request a duplicate copy.

Student Related Tax Forms

Students will receive the 1098-T for tuition payments, in addition to any employment related tax forms, electronically. Currently, the 1099-INT for the Prepayment Incentive Credit (PIC) Plan is distributed by paper copies only.
