W-2

If you wish to receive an electronic copy of your W-2 instead of a paper copy, you will need to consent to receiving an electronic copy through the Self Service portal. View Instructions for performing this task.

What is a W-2?

A W-2 is a form that the IRS requires that employers issue to report wage and salary information for employees. A W-2 also reports the amount of federal, state, and other taxes withheld from your paycheck from January 1 to December 31 of each calendar year. This form is used to file your taxes.

Who receives this form?

All University employees who receive any salary, wage, or other compensation within a calendar year.

How will this form be sent?

  • Current employees can consent to electronic delivery through Self Service. Once electronic delivery has been chosen, no paper forms will be produced. Once electronic delivery has been chosen, you continue to have access to current and historic W-2s unless you withdraw your consent. 
  • For employees who have not consented to electronic delivery, a paper copy of the W-2 will be forwarded via campus mail on January 31. 
  • All former University employees will receive a paper form delivered via first class U.S. mail to the most current address on file.

When will this form be available?

All forms are released by January 31.

Why are my wages, tips, and other compensation (Box 1) different than my Social Security (Box 3) and Medicare (Box 5) wages?

Pre-tax benefits that are covered under IRC Section 125 are non-taxable for Social Security, Medicare, federal and state/city income tax purposes. 403(b) contributions are non-taxable for federal and state/city income tax purposes only. 

The following are common reasons that the amounts reported in Boxes 1, 3, and 5 may differ:

  • 403(b) before tax contributions are tax deferred for federal and state/city income tax purposes, and therefore, are not included in the amounts reports as federal taxable wages and state taxable wages. These amounts are, however, reported as Social Security and Medicare wages. Therefore, you will see a larger amount reported in Social Security and Medicare wages than in state/city and federal taxable wages.
  • Social Security wages are limited to $118,500 for the calendar year 2016.

What if my name and/or Social Security number is incorrect on my W-2?

Payroll will correct and reissue your W-2. Corrected W-2s are called W-2cs and are processed until February 28. If you find your name and/or Social Security number is incorrect, please contact Tax Forms as quickly as possible. You will be contacted when the correct W-2c is available for you.

What if the wrong address is listed on my W-2?

If the wrong address is listed on your W-2, a corrected copy will not be created or distributed. Please update your address information in CedarInfo.

What if I need a replacement?

Employees can either consent to electronic viewing of the document via Self Service or contact Tax Forms to request a duplicate copy.

I am a parent. Can I receive a copy of my child’s W-2 Form?

No, due to the Privacy Act of 1974, only current or former employees can request a copy of their W-2.

1095-C

If you wish to receive an electronic copy of your 1095-C instead of a paper copy, you will need to consent to receiving an electronic copy through the Self Service portal. View Instructions for performing this task.

What is a 1095-C?

The form 1095-C includes information about the health insurance coverage offered to you by Cedarville. Form 1095-C Part II includes information about the coverage, if any, the University offered to you, your spouse and dependent(s). Part III lists the covered individuals. This form is necessary to file your taxes.

Who receives this form?

All full-time employees of Cedarville for one or more months within a calendar year.

How will this form be sent?

  • Current employees can consent to electronic delivery through the Self Service portal. Once electronic delivery has been chosen, no paper forms will be produced and you will continue to have access to current and historic 1095-C's unless you withdraw your consent. 
  • For employees who have not consented to electronic delivery, a paper copy will be forwarded via campus mail by January 31. 
  • All former University employees will receive a paper form delivered via first class U.S. mail to the most current address on file.

What if my name or the name of my dependent(s) and Social Security Number is incorrect?

If you find your name and/or Social Security number is incorrect or your dependent(s), please contact Tax Forms as quickly as possible. Payroll will correct and reissue your 1095-C. You will be contacted when the correct 1095-C is available for you.

Student Related Tax Forms

Students will receive the 1098-T for tuition payments, in addition to any employment related tax forms, electronically. Currently, the 1099-INT for the Prepayment Incentive Credit (PIC) Plan is distributed by paper copies only.

What is it for?

The 1098-T is a form designed by the Internal Revenue Service (IRS) for educational institutions to report qualified tuition, fees, scholarships, and grants. These forms are to be used when completing individual federal tax returns and are not to be taken as tax advice.

Who receives this form?

All students enrolled in Cedarville the previous calendar year.

How will this form be sent?

  • All current students have access to current and historic 1098-Ts through the Self Service portal.
  • Former students who consent to electronic delivery through the Self Service portal can access all 1098-T documents electronically and no paper forms will be sent. Once electronic delivery is chosen, you continue to have access to current and historic 1098-Ts unless you withdraw your consent. 
  • Former students who did not consent to electronic delivery will have a paper form sent through first class U.S. mail to the most current address on file.

When will this form be available?

Why is Box 1 blank on my 1098-T form?

Box 1 will be blank for all Cedarville University students. The IRS requires all educational institutions to select a reporting method and Cedarville has chosen to list all amounts billed for qualified tuition and related expenses in Box 2.

Please Note: Due to the reporting method Cedarville University has chosen, all tuition expenses are reported as to the calendar date they were billed and all payments are reported as to the calendar date they were received.

What charges are considered "qualified" or "eligible" for reporting in Box 2?

Eligible or qualified charges are any tuition charges plus laboratory fees, graduation fees, and enrollment fees. All other charges to your student account, including room and board, are not considered eligible charges.

What does an amount in Box 5 mean?

Box 5, Scholarships and Grants, shows the net amount of certain forms of educational assistance that was received or applied to your student account between January 1 and December 31 of each calendar year, regardless of the term that funds may have originally be designated for.

My name and/or Social Security number are incorrect on my 1098-T? How can I have this corrected?

1099-INT

What is it for?

Form 1099 INT reports interest earned from a student's account during a calendar year.

Who receives this form?

All students who received more than $10 Prepayment Incentive Credit (PIC) plan interest from their student account balance during the previous calendar year.

How will this form be sent?

  • Current students who have removed the Family Educational Rights and Privacy Act (FERPA) privacy indicator on their account will have a paper copy forwarded to their campus mailbox.
  • Currently enrolled students who have not removed the Family Educational Rights and Privacy Act (FERPA) privacy indicator on their account will have a paper copy forwarded to their campus mailbox. 
  • Former students will have a paper form sent through first class U.S. mail to the most current address on file.

When will this form be available?

All forms will be released by January 31.

My name or Social Security number are incorrect on my 1099-INT. How can I have this corrected?
