The employee performance evaluation process utilizes an online system or a PDF fill-able document. The web form is accessible via the link below or through CedarInfo under the Employee Services section. All methods used to conduct an employee evaluation should be completed by providing an original printed copy with appropriate signatures to University Human Resources. Please review instructions for additional information.
- Online Performance Evaluation Form: This preferred method allows the supervisor to access all employee evaluations that they are responsible to complete. This method also provides the employee electronic access to the performance evaluation once the process is finalized.
After an online performance evaluation is marked as "completed", a "print evaluation" button will display at the bottom of the evaluation.
When "print evaluation" is selected, all text within the evaluation text boxes will format and print properly.
- Staff Evaluation Form: PDF fill-able electronic document should be used to evaluate all staff persons who have no official supervisory duties.
- Manager Evaluation Form: PDF fill-able electronic document should be used to evaluate all staff persons who officially supervise permanent staff employees.
Download and save PDF documents to a network drive to ensure changes are saved.
